Description

PROCAREFUL project aims to address existing gaps in home care accessibility and timely delivery by upscaling existing social care services and targeting the potential for preventing cognitive and physical decline. Partners will develop a strategy for the digital transformation of home care, test the deployment of a hybrid care approach supported by digital solutions in five pilot actions, and elaborate action plans for the investment implementation. SHINE is responsible for delivering an evaluation model and an impact assessment of the activities as well as policy reccomendations
